True to the nature of a nightmare, your workspace now becomes a terrifying "gore" (bloody) setting. But the biggest change lies in the inhabitants. The impersonators now not only mimic humans but also grotesque creatures and demons, delivering an incredibly haunting "body horror" experience. For example, the milkman Francis Mosses now appears in the form of the Yog Sothoth entity or characters inspired by other horror games like Xuchilbara Lobsel Vith (designed to look exactly like Nurse from Silent Hill).
Gameplay changes: Pay a heavy price for every mistake
In regular Arcade Mode, you can survive until the end of your shift before being killed by a Doppelganger. But in Nightmare Mode, the punishment mechanism is far more brutal:
- Instant Game Over: Any doppelganger that successfully gets through the gate will immediately attack you, resulting in an instant game over. This turns Nightmare Mode into a race for survival with no room for hesitation.
- The Entity's Wrath (The Purple Entity): If you make the mistake of summoning D.D.D. to eliminate an innocent "real" resident, the consequences won't be immediate. The first time you summon the wrong person, a purple face with deep-set eyes and a gaping mouth—known as "The Entity"—will appear on the opposite wall. If you summon the wrong person a second time, the entity will immediately attack and finish you off.

Nightmare Mode features characters you can't deal with using common sense:
- V.I.P. Peach: A big surprise for veteran players. In regular mode, Peach is an extremely dangerous imposter. However, in Nightmare Mode, Peach is a rare V.I.P guest you can safely let into the building.
- Mask Ghost: This character can appear multiple times in a row during a shift, a characteristic that in regular mode would certainly be considered a doppelganger, which is a term used to describe a character that mimics another.
- The Nightmare: This character is the game's "final boss." He always appears at the end of a shift and demands you play a game of the best out of three formats. Don't be foolish enough to press the D.D.D. button! If you do, the Clown will kill Agent D.D.D., display his head on a stick, and still force you to play cards. The only way to complete the game is to defeat him at the card table.

Chester – the hippie who asks riddles – has now become Evil Chester with incredibly difficult questions. To achieve success, you must answer correctly questions such as the following:
- The last name of the person behind the most famous mask on the internet: Fawkes.
- The largest number written in traditional Roman numerals: MMMDCCCLXXXVIII (equivalent to 3888).
- The name of the hairiest lieutenant in World War II: WOJTEK (a bear).
- The temple where a god once caused the greatest pandemic: ZUL'GURUB (the blood leak event in the game World of Warcraft).
- The multicellular organism capable of replicating everything it touches: SCP-038 (or SCP038).
- Operation Trojan Horse Secret: During gameplay, you might encounter a Hooded Man who gives you a piece of paper containing a code.
